Anastasia “Stassie Baby” Karanikolaou, Kylie Jenner’s longtime best friend and frequent collaborator, is opening up about reversing one of her most famous cosmetic procedures — her Brazilian Butt Lift. The 28-year-old influencer shared her journey across YouTube and TikTok, detailing the lead-up to her surgery with Beverly Hills plastic surgeon Ashkan Ghavami, MD and the emotional reason she decided to “de-BBL” after a decade.

“This Was a Mistake I Made Super Young”

In her YouTube vlog filmed before surgery, Stassie said she’d been struggling with her decision for years. “It was a decision I made when I was so young that I didn’t realize the consequences of my actions at that age,” she shared. “I think I’m a completely different person at 28 than I was at 18. I’ve always been vocal that I knew I had messed up. I’m just happy I’m finally making the decision to feel more comfortable in my skin.”

What a BBL Reversal Actually Involves

A Brazilian Butt Lift (BBL) reversal isn’t a one-size-fits-all surgery, it typically combines fat removal, redistribution  and tissue tightening to restore balance and proportion. Depending on the patient’s goals, the procedure may involve liposuction to reduce excess fat, skin tightening technologies like radio frequence and fat grafting to reshape rather than completely flatten the area.

Tightening and Refining, Not Removing

In her pre-op vlog, Dr. Ghavami explained that this surgery wasn’t about erasing the past but refining proportions. “We’re basically tightening tissue, and because she’s healthy and has great muscle tone, we can do some tricks in the operating room to tighten that skin that’s loosened. Gravity always wins,  the buttock is one of the first places that gets loose,” he said. “We’re going to do what’s called Quantum, which is radio-frequency tightening, and then some fat manipulation and relocation to volumize and lift while reducing overall size.”

He compared the approach to repositioning fat under the eyes rather than removing it, allowing for smoother, more natural contours. “We’re lifting, tightening, making smaller — all the tips and tricks,” he added. “She’s taking great care of herself and she’s going to keep it going post-op with photobiomodulation and red-light therapy.”

Feeling More Herself

After surgery, Stassie joked from recovery that she was “eating Greek salad and recovering from my BBL removal,” adding that while it’s “still there,” she simply wanted to look “more proportionate to this body.”

Responding to online speculation that she was “following trends” by reversing the popular 2010s-era procedure, Stassie set the record straight on TikTok. “The reason I’m sharing my experience is because I wish I had someone that showed me the not-glamorized side of plastic surgery when I was younger,” she said. “It’s actually something that made me more insecure and made me feel unauthentic to me as a human being. You shouldn’t change your body to follow a trend. Do what makes you feel comfortable, not what you think you should do.”
